How do I use this program?

To move between the screens, please answer the questions and use the buttons that will appear on the bottom of the screen.  You may also use the Navigation Menu on the left-hand side of your screen to return to a previous section. Do Not use your browser’s back button! If you do, you may lose information you have entered and need to start over. 

If you want to leave before you finish, just close your browser.
